It is very important that you complete initial intake paperwork prior to your appointment time as this will result in the most effective use of your time.

New Clients: Prior to your first appointment, a link will be emailed to you which will allow you to complete and submit the initial Intake paperwork through our secure client portal.

Returning Clients: You can access the portal with your username and password to view account information, send secure email messages, and to view upcoming telehealth or in-person appointments. Click on Client Portal  to log in.
